Hi

If you are asking about a PT. The PT will read the chart and referral papers before hand. PT will ask questions and will do an evaluation of the patient needs, will develop a Rx plan. Some PT do some treatment and home program on the first day.
If you are asking about a client/ patient. The patient should bring any testings or referral that pertain to this condition. Patient should have a list of meds and history of past illnesses.

Hello,

If you are a PT, then it mainly depends on which setting you are working. Reading the charts and any other information available on the file is helpful to have a background about your patient. An evaluation is done after basic observation followed by treatment depending on the setting. Make sure to perform outcome measures as part of the evaluation.

As a patient, please make sure you carry your ID card, insurance card, referral paper and any other reports pertaining to the problem you are seeking therapy for.
