Is it still possible to get into a top 20 school with a 3.8 gpa?

For context in 9th grade I got a 4.0 gpa and for this year I got a 3.6 gpa for semester 1 despite the fact I had a 4 for all my classes except for marketing. I got a 3.2 which is considered an A at my school. But because I enrolled into marketing class late which is not my fault at all my teacher decided to give me 0.25/0.50 credit even though I made up the past work. My counselor decided to change my class before the end of the semester which I was so confused about and I wasn’t allowed to change my class. The way my school calculates gpa is extremely weird and unfair. There’s no way I got a 3.6 gpa for the first semester that doesn’t seem right. I am in 10th grade.

I selected my classes for next year and chose to do the IB program. I am starting to regret that choice. I would rather do partial IB. Do you think It’ll look bad if I drop out of the IB program to do partial IB? My goal is to get into a top 20 school. Also my school is an IB school so they do not offer AP classes. I plan to study to become a #physician after I graduate from high school. I want to attend a "good" #medical-school, but I don't know if I'll be able to get into one if I attend a #public-school. I'm a junior in high school. I have a pretty decent GPA and I have all A's. I've done a tone of program like internships abroad at hospitals and labs and I'm currently doing a leadership program with Princeton and a mentor-ship program with a pharmaceutical company called Novartis. I'm in 5 clubs and I hold leadership positions in 3 and will be holding leadership in all 5 next year. I currently take 3 AP classes and I probably will take 5 next year. As a junior in high school, I am starting to prepare to apply to colleges. My school is quite small (a total of 175 students only in my grade). Although this is so, previous graduates of my high school have gone out to Columbia, Stanford, NYU, Cornell, Yale, and other elite universities. I am in the top tenth percentile of my school, but I am not so sure if I should apply to Ivy Leagues or colleges with much lower percentage rates.
