What is considered a 4.0 GPA? Is it anything above a 95 average? or is it a 100 average?

There are several scales and converters available online where you can directly compare the GPA in different terms. Also, if you are looking in terms of a particular college/university, you can try asking them specifically. Be careful of the online links you select for comparison, you will find different information everywhere. It will not give an exact answer but more of an estimate.

A 4.0 GPA is anywhere from a 93-100.

I think that it varies by the college or university because there is no standard way to calculate the GPA so each school may use a different methodology. If you need a definitive answer I would contact each school your interested in attending.
