I haven't been a social worker, but I have worked with many in my history as a health care worker and am familiar with their role. The paperwork you have to do will vary by where you work and what type of clients you work with, for example the paperwork in a hospital is different than the paperwork for child protective services. Generally speaking Social workers spend time in hospitals doing counseling with patients and then will put a note in the medical chart describing what was discussed, what areas they are addressing (for instance emotional adjustment to a new medical condition), and their professional opinion on how the patient is progressing. Finally they will note a plan, what they have given the patient to work on during their own time and what they plan to do in the next meeting time. If it is the first time you meet with someone your notes will be about your assessment of the patient, what services you provide that can help them.

Social workers work in many other settings and can have a wider scope than counseling, it is a field with a lot of flexibility to change when your interests change.

It truly depends on what type of social work you wish to go into. One you might want to familiarize yourself with is the biopychosocial assessment. It may look differently based on the agency, but there are key points to all. It's an assessment that gives you a snapshot of the person based on their physical self, their mental self and their social self. It can identify areas that need to improve before success and progress are possible.

That it one tool I have seen across several different fields with slightly different names.

In addition to completing a treatment plan, based on your agency you may have to complete monthly billing forms and occasional utilization reviews. The utilization reviews require the therapist or agency to contact the medical insurance company to gain authorization for treatment. This would require reviewing treatment plan, biopsychosocial assessments, and therapy notes.
