To be involved in sports medicine, the options are personal trainer, physical therapist, occupational therapist (less so), and physician. To be a physician, one has to complete college with excellent grades and letters of recommendation so you may get into medical school (international medical schools are an option, as are MD and DO programs). In medical school one "rotates" through many obligatory rotations and some elective rotations. It is on these rotations that one begins to refine goals and decide what area of medicine seems most attractive. Within sports medicine, one could be a physiatrist, an orthopedic surgeon, or do pediatrics or internal medicine. All of these residencies are usually followed by sports medicine fellowships.
Because it is a many-year, daunting task to become a sports medicine physician, you could decide the somewhat easier course of becoming a therapist is more to your liking.

I would say that you would have to go to med school. Once you finish med school and start to apply for residency that will be where you can decide what kind of doctor you want to become. You can apply for a sports medicine residency if you want to be a sports injury surgeon. There is also the choice of doing a fellowship after residency to further your knowledge for a sports medicine surgeon.
