Couple things in making this decision- if you would like to become a physician assistant, take the course work you would take as a pre medicine student. Most schools do not have a "medicine" major but instead a "pre medicine track." As a "pre med student" you would major in something- most major in biology, chemistry, physics or on occasion majors which are non science based like history, women's studies, psychology, soiciology. One you've graduated from college, you can decide you'd like to be a physician assistant or go to medical school. That gives you options.

If you would like to be a nurse practitioner, then you will need to major in nursing. Once you graduate nursing school, you will have a Bachelors in nursing. You can either start working as a nurse or go immediately back to school and get your masters or doctorate of nursing. Only after you have obtained your master's degree will you be a nurse practitioner.

Many schools allow you to transfer into the nursing program, so if you decide the pre med track is not for you then you can transfer. Many of the initial prereqs for nursing like biology, microbiology, english, math are the same for pre med and nursing.
