I may not have a background in medical science, but I've learned in the business world—and this holds true everywhere—that hard work, gaining knowledge, and showing what you know in the right way can really help you shine. A little bit of luck also plays a part. Networking and being open to sharing your dreams and aspirations with others always gives you a boost.

You can stand out by finding what you are passionate about and good at, and distinguishing yourself from others by being yourself. You don't have to be quirky. You don't have to win some prestigious award. You have to be able to convince others that you care about something, and are working to make a difference.
As an athlete, it does certainly help your resume to have been a team captain. When I look at application letters I am looking for athletes that are interested in helping others be successful athletes. I don't actually see how being a state champion makes you a better doctor. I do see that if someone helps train others - perhaps those who would struggle to get that help - I am thinking "hey, that person will probably be a great physician!"
One other thought is if you have any other passions besides athletics, you may want to highlight that. Are you a musician, dancer, artist, gardener, chef...?
Distinguish yourself by being a thoughtful, caring, human that applies their gifts to benefit those around them.
