There is no one right answer to this question. It is important that you take classes you enjoy. Yes, you have to take certain prerequisites, but maybe you have always wanted to learn about economics or geology, or business administration... As for extracurricular activities, you need to do what brings you joy. During college I was able to enjoy music, theater, and intramural sports. You do need to get excellent grades, so as you sign up for courses and majors you should know that you can get an A in that course (with the significant effort needed to get As in college classes).
You can't really be clinical during college. It is good to find shadowing opportunities that help you tell your personal story about how such experiences have led you towards wanting to be a physician.
but it isn't spending hours with patients. It can be helpful to demonstrate research lab experience, so if you are able to secure an internship or other opportunity that could help you. Professors who have students in their labs often write the most compelling letters of recommendation.
Don't just check boxes of to-dos. Medical Schools are looking for individuals who can demonstrate the ability to master subject matter, think with alacrity, lead teams, and demonstrate care and compassion.
