Different types of applications may require different emphasis, if it's for graduate school, for example, they might focus more on your academic abilities and potential as a scholar, while for a job, they might focus on your communication skills and leadership potential.
You may also give them your resume or CV. This is one of the most helpful things you can provide. A resume outlines your experiences, skills, and accomplishments, making it easier for your teacher to see your strengths and how they align with the goals of the program or position you're applying for. Even though your teacher may know you from class, they may not be aware of all the extra-curricular activities or jobs you've done that could be important to mention.
Also, you can include a list of courses or projects you've done with them. If your teacher has taught you in multiple classes, or you've worked closely on a project or research paper, remind them of these details.
