I'm pursuing a career in ultrasound, but I'm undecided on which specialty to choose (like OB/GYN, cardiac, or vascular). What's the best way to figure out which ultrasound specialty is the right fit for me #Spring25?

I'm currently a college student pursuing a career in diagnostic medical sonography. I'm really interested in the heart and how it works, so cardiac ultrasound stands out to me. But I also think I’d enjoy working in OB/GYN and being part of such special moments in patients’ lives. I’d love some advice from sonographers who’ve worked in these specialties or faced a similar decision!

I recommend you try to spend time with different ultrasonographers to see what feels right to you. I always enjoyed working with echcocardiographers. There are even those who bridge your two top choices by becoming experts in fetal echocardiography! Remember that you are not allowed to actually diagnose officially. Your job would be to obtain high-quality images that allow a radiologist or cardiologist to make the diagnosis. In any case, if you become an ultrasound clinician, you will be highly interactive with patients and their families. It can be a very rewarding field in which you can go home at the end of the shift feeling that you helped people - and that's a great feeling.

I recommend shadowing someone who has the same profession you are interested in. I also think it would be a good idea to shadow other jobs just to make sure you are set on what career path you wish to choose.

Here are a few things to consider from sonographers who’ve been in your shoes:

🫀 Cardiac Sonography (Echocardiography):

If you love the heart and want to dive deep into anatomy, physiology, and how everything functions dynamically, echo is a great choice.

You’ll often work with patients who have or are being evaluated for heart disease, so it can be fast-paced and more clinical in nature.

There's often more complexity and a steeper learning curve, especially when interpreting valve function, blood flow, etc.

Great for those who enjoy problem-solving and detailed scanning.

OB/GYN Sonography:

If you’re drawn to being part of emotional, meaningful milestones—like pregnancy confirmation, gender reveals, and seeing a baby grow week to week—OB/GYN might fulfill you in a more personal way.

It involves both routine and high-risk pregnancies, and you’ll need a strong sense of compassion and emotional resilience, especially in difficult cases.

There’s also a lot of variety in scanning beyond just obstetrics—gynecological exams, fertility-related imaging, etc.

Advice from sonographers:

Many say shadowing in both areas (if you haven’t already) was the most helpful part of making their decision.

Some start in one specialty and transition later—it’s not always a final choice!

If you find yourself energized by technical detail and complex pathology, echo might be your path.

If you find joy in patient interaction and emotional connection, OB/GYN may speak to your heart more.

No matter which you choose, your work will have a huge impact on patients’ lives. Keep exploring and trust that your passion will guide you to the right fit. You’ve got this!

I think I might be uniquely overqualified to answer this question. I am currently an echo sonography intern who is working at an OB clinic. (How? The clinic offers OB services, but the owner teaches students echo in the back room.) Some differences I've noticed and seen are mostly in the lifestyle. If you like talking to patients and young (possibly loud) kids who may be brought to appointments, then OB might be great. It can also have its bad moments when something is wrong with the baby. This is more common in high-risk OB or MFM. Echo, you don't have to talk as much to patients. In fact, during an exam, the patient must be quiet for the duration of the ultrasound. It requires a VERY steady hand for methods such as M-mode. If you've found your grip is not the best or you're a bit shaky, these are things you can train and work on, but they may be a deterrent at first. Shadowing is an option, but many sonographers don't really get shadowing opportunities. Most schools offer internships, so try to experiment there.

For your registry exams, OB and Echo are very different. OB will mostly be identifying what happens during what stage of pregnancy and pathology, while the echo registry exam will have a lot of ultrasound physics due to the blood flow.
