While I'm not a nurse, I did some research that might help you figure out how to become a neonatal nurse in South Africa:
1. You will need to go to school and get a degree in nursing. It looks like a Bachelor's in nursing would be the easiest way to become a professional nurse, which is important because this status is needed for step 2.
2. Register with SANC (South African Nursing Council)
3. From there, it really depends. It looks like some jobs are willing to train new nurses while others want you to have multiple years of neonatal (or similarly relevant) experience. I'd recommend looking at job descriptions for places and roles you're interested in to see what they are looking for in a candidate.
Another thing to note is that this would give you the title of a "professional nurse" but not a specialist. For the specialist title, you would need a postgraduate diploma
